Notice on further clarification of Health Code and Health Declaration Form application requirements

At present, the COVID-19 epidemic situation is still severe, and the risk of cross-infection from international travel remains high. In order to consolidate the hard-won results of epidemic prevention and control, and ensure the health and safety of those whose travel to China is absolutely essential, the Chinese Embassy in Indonesia would further clarify the application requirements of Health Code with "HS" symbol (for Chinese citizens) and Health Declaration Form with "HDC" symbol (for foreign citizens) for all passengers travelling to China, and will therefore implement the following measures starting 0:00 February 24, 2021:

1. All passengers departing from Indonesia to China should take direct flights. The Chinese Embassy and Consulate in Indonesia will no longer issue Health Code or Health Declaration Form for passengers transiting to China via a third country.

Simultaneously, the Chinese Embassy and Consulate in Indonesia will no longer accept applications for Health Code or Health Declaration Form of those who depart from a third country and transit through Indonesia to China.

2. All passengers going to China should be isolated in a single room at least 14 days before departure and monitor their health conditions as required. Those who have been in isolation for 14 days and have normal body temperature and no respiratory symptoms may proceed to complete nucleic acid tests and serum IgM and IgG antibody tests at two different designated institutions within 48 hours before boarding with the assistance of the relevant airlines. The Chinese Embassy and Consulate in Indonesia will decide whether to issue Health Code or Health Declaration Form based on the regulations and the test results (numerical value).

3. All China-bound passengers whose nucleic acid test and serum IgM and IgG antibody test results from designated institution shows any item to be positive will be deemed infected before, except those who hold the certificate of vaccination.

Passengers who have been infected should be tested twice at designated institution (at least 24 hours apart) in accordance with the requirements of the Chinese Embassy in Indonesia. When the test results meet the requirements and isolation period of a single person living in a single room for more than 14 days is completed, the passenger may proceed to complete nucleic acid and serum antibody tests at two different designated institutions within 48 hours before boarding with the assistance of relevant airlines. When the test results meet the requirements, the passenger may submit an application of Health Code or Health Declaration Form to the Chinese Embassy in Indonesia.

For those who cannot confirm a recovery, the person should appropriately provide a diagnosis certificate containing CT or X-ray results of the lungs issued by the local hospital.

4. When applying for a Health Code or Health Declaration Form, in addition to the nucleic acid and serum IgM and IgG antibodies negative test results, all China-bound travellers should provide the following materials:

1) Work certificate issued by the employer (stamped with the official seal of the company and signed by the legal representative or its authorized personnel);

2) Indonesian residence permit, visa or entry stamp;

3) Other documents required by the Chinese Embassy in Indonesia.

5. Passengers who depart from East Timor or other countries to Indonesia via land ports for transit to China,should first undergo nucleic acid test and serum IgM and IgG antibody test at the place of departure, and apply for a Health Code or Health Declaration Form from the Chinese Embassy in the place of departure. If there is no record of application, the Chinese Embassy in Indonesia will not review and issue Health Code or Health Declaration Form.

6. Passengers taking flights to China should strictly abide by the epidemic prevention and control regulations and submit relevant materials truthfully. Any person found to conceal the condition, tamper the test certificates, not comply with the isolation requirements, or fill in false information, will not be able to obtain the Health Code or Health Declaration Form. Those who violate the law will be held accountable.
